Radiology resources

Primary-source summaries, not a substitute for the full guideline or local policy. Version matters.

1. Fleischner pulmonary nodules2017

Incidental nodules in adults. Solid single nodules: <6 mm, no routine follow-up at low risk and optional CT at 12 months at high risk; 6–8 mm, CT at 6–12 months with possible/required 18–24-month CT by risk; >8 mm, consider CT at 3 months, PET/CT, or tissue sampling. Does not apply to screening, immunosuppression, or known primary cancer.

2. ACR TI-RADS2017

Thyroid US point system using composition, echogenicity, shape, margin, and echogenic foci. The linked independent calculator implements the 2017 ACR point system and size-based FNA/follow-up guidance; keep the official ACR chart beside it for source verification.

3. O-RADS US / MRIUS v2022

Adnexal risk stratification. US risk bands: O-RADS 2 <1%, 3 = 1–<10%, 4 = 10–<50%, 5 ≥50%. A physiologic follicle is ≤3 cm. In O-RADS 5, ascites/peritoneal nodules or high-risk lesion features prompt gyn-oncology management. Use the linked tables for morphology, color score, menopausal status, and follow-up.

4. BI-RADSv2025

Standard lexicon and final assessment for mammography, breast US, MRI, and CEM. Current v2025 materials are the authority; use the ACR lexicon forms and manual rather than a copied category table because modality descriptors and guidance changed.

5. Lung-RADSv2022

Lung cancer screening CT only. Solid baseline nodules: <6 mm = category 2/annual LDCT; 6–<8 mm = category 3/6-month LDCT; 8–<15 mm = category 4A/3-month LDCT; ≥15 mm = category 4B/further diagnostic evaluation. New and growing nodules use different thresholds.

6. LI-RADS CT/MRIv2018

For patients at high risk for HCC. LR-5: 10–19 mm with nonrim APHE plus nonperipheral washout, or APHE plus threshold growth, or APHE plus both washout and enhancing capsule; ≥20 mm with nonrim APHE plus washout, capsule, or threshold growth. Threshold growth is ≥50% in ≤6 months.

7. PI-RADSv2.1, 2019

Prostate MRI likelihood of clinically significant cancer. DWI is dominant in the peripheral zone; T2W is dominant in the transition zone. A category-4-type lesion becomes category 5 at ≥1.5 cm or with definite extraprostatic extension/invasive behavior. DCE mainly upgrades a peripheral-zone DWI score 3 when positive.

8. Bosniak cystic renal massesv2019

CT/MRI classification. Thin wall/septa are ≤2 mm. IIF includes a smooth 3-mm enhancing wall or septa, or ≥4 smooth thin enhancing septa. III has enhancing thick wall/septa ≥4 mm or irregular enhancing thickening. IV has an enhancing nodule: any size with acute margins, or ≥4 mm with obtuse margins.

9. SRU simple adnexal cysts2019 update

Asymptomatic simple cysts. Premenopausal: report >3 cm; follow only when >5–7 cm, using 7 cm when characterization is excellent. Postmenopausal: report >1 cm; follow only when >3–5 cm, using 5 cm when characterization is excellent. A 10–15% decrease can end follow-up.

10. ACR Appropriateness Criterialiving guidance

Evidence-based imaging selection by clinical scenario, with modality ratings, radiation level, and supporting evidence. Search the live portal because topics are updated on a rolling basis.

11. NeuroQuant age-related atrophyeducational PDF, 2018

A one-page educational interpretation table for the NeuroQuant Age Related Atrophy Report. It compares hippocampal volume, hippocampal occupancy score (HOC), and superior/inferior lateral-ventricle volume patterns, with example interpretations ranging from normal to possible local or global ex-vacuo dilatation. This is not a society guideline and gives no numeric cutoffs.
